Government civilian employees can contact the Employee Assistance Program for assistance. If you think you’ve recently been a victim of identity theft, your EAP can help you take the necessary steps to minimize the damage and protect your credit rating. The EAP can also provide a kit full of information and resources on protecting yourself from identity theft.

Lost/stolen IDs must be reported
There are new procedures in place for reporting lost or stolen Common Access Cards at Hill Air Force Base.
The cards, used by Team Hill members to enter the base and access computer networks, are controlled documents that contain personal information.
As soon as the discovery is made that a card has been lost or stolen, members must report the issue to the 75th Security Forces Squadron Pass and Identification Office or Emergency Communications Center at 801-777-3056.
They must then visit the 75th Security Forces Squadron in Building 408 and fill out an Air Force Form 1168, explaining the circumstances surrounding the lost or stolen card.
The Form 1168 is required before users can have a new card issued by customer service at the Force Support Squadron in Building 430.
For more information, contact the 75th Force Support Squadron Customer Support at 801-777-1845, option 3.
