HILL AIR FORCE BASE, Utah — The 75th Medical Group Family Health Clinic will be conducting school, sports, and scouting physical exams from 2-4 p.m. on the following dates: Thursday, July 20; Thursday, Aug. 10; and Tuesday, Aug. 15.

Physical exams will be conducted by clinic health care providers by appointment only and solely for the purpose of completing the physical exams. We may ask you to reschedule for routine medication refills and chronic/acute medical conditions. Appointments may be made by calling the Central Appointment Line at 801-728-2600 or via the TRICARE Online Patient Portal account.

Beneficiaries are asked to fill out and bring the necessary physical exam forms required by their respective schools or organizations to their appointments. Please also bring a copy of the child’s immunization record to the appointment.

A physical exam may not be necessary if a dependent has had an annual physical exam completed within the last year. If a physical exam has been completed within the last year, a record review in lieu of an appointment can be done. Simply fill out the forms for the child’s school/organization and drop it off at the Medical Group’s Family Health Clinic front desk.

Please allow three duty days for completion. Physical Exam forms for a record review may also be sent via the TRICARE Online Patient Portal secure message option (formerly MiCare).
