Hill clinic set to assist with Utah school physicals

In response to parent feedback, the 75th Medical Group is providing appointments for children with their primary-care provider for annual wellness visits and school physicals. The question you should be asking is whether or not your child needs a school physical this year.

The Utah Department of Health requires children enrolling in school for the first time, changing school districts, or participating in sports to have a physical in the 12 months prior to the start of the school year. 

While Utah forms are preferred, comparable forms from other states are acceptable if completed within the past year. Key Utah forms are available online. The Initial Physical Exam Form can be found at www.uhsaa.org/forms/forma.pdf and Form B (used when Initial Physical Exam Form is already on file) can be found at www.uhsaa.org/forms/formb.pdf. 

If your child has had a well-child visit or school physical in the past 12 months, the forms can be dropped off at the clinic and will be completed by our staff within three duty days. 

The forms can also be emailed to your child’s provider via the MiCare secure messaging system. To prevent exposure of protected health information, forms emailed in any other system cannot be completed. If you have not signed up for MiCare, you can register online at https://app.relayhealth.com/RegistrationV2.aspx?bid. 

Through MiCare you can communicate with your health-care team, view lab results, have prescriptions renewed, and make appointments. If you have questions about MiCare, contact Capt. Chris Stewart at 801-586-9570 or 1st Lt. Kelly Sampson at 801-586-9807.

If your child needs an appointment, call the appointment line at 801-728-2600 or make a request via MiCare. Keep in mind these appointments are intended to address your child’s school or sports physical and preventive health-care needs. We will make every effort to complete medication refills and address other concerns, but you may be asked to schedule another appointment for additional issues to ensure each concern can be properly addressed. 

If your child doesn’t need a school physical, he or she should still receive an annual wellness visit. We recommend performing these around their birth month so that it’s easy to remember.

You will also need to ensure your child’s immunizations up to date. The immunization clinic is open 7 a.m. to 4 p.m. for walk-in patients or to obtain records. Records may also be requested via MiCare. As a reminder, scheduled immunizations are due at 4 and 11 years old.
